This is the typical grenade launcher for russian and chinese forces in video games. Other games might give it a different name, example: Battlefield Bad Company 2 calls this grenade launcher a "SMK 40mm"

The MP5 has a ton of variants and each can be modified. In games usually you find a submachine gun close to the MP5 A4 or MP5 A2, if a game just says "MP5" we reference to the MP5 A4 then. The variants use the calibers 9x19mm, 10mm Auto (for MP5/10) and .40 Smith & wesson for the MP5/40 gun.

In real life, you can't see the warhead/projectile while it flies. See the movie to get an impression, how fast a rocket is in real life. Firing inside buildings: Yes. As you can see in the videos, it is possible to fire that weapon even in very narrow spaces.Also this video shows the different modes (against structures or to breach walls ("mousehole mode")).
